Nutrition Assessment and Referral
Have you ever completed a nutrition assessment by a professional dietitian? This evaluation will determine your nutrient status and your risk or status for nutrition-related diseases and conditions such as high blood pressure, cardiovascular disease and diabetes. You will be provided with a copy of your nutrition profile as well as appropriate referrals to healthcare professionals or services.
Cost: $75.00 for 60-minute assessment
Disease Management Consultations
These sessions are designed to educate and guide you in the nutrition management of your condition or disease. The number of sessions needed are generally determined after your initial nutrition assessment has been completed. These sessions can span a week or a few months. The number of sessions needed vary, but typically at least 3-6 sessions are necessary to establish the best long-term management practices and diet/lifestyle changes.
Cost: $50.00 for 30-minute follow up and recommendations

Payment
The initial nutrition assessment and referral session is typically an out-of-pocket expense for the client. However, if the client has a diagnosis of diabetes or kidney disease it can be covered by insurance such as Medicare/Medicaid.
Sessions must be paid in full prior to the session date if insurance coverage is not available.
Physician Referral Form
The next time you see your doctor, make sure you have them sign our referral form so we can bill your insurance if you have a coverable diagnosis.
